The essential components of a complete radiology request form include: the who, what examination will be performed, and why the examination is being conducted. All request forms have clearly marked fields, for ease of completion by the requesting physician as well as ample space for peritnent clincial history. The request form must be completely filled out for the examination to be conducted. The main components are:

1. Patient Identifiers: Name, social security number, etc.

2. Examination to be Conducted: The radiologic examination to be conducted on this particular patient

3. Clinical History: Pertinent clinical history

4. Reason for the Examination: Why the examination needs to be conducted
