Fellowship Applications for Graduates

You can e-mail or mail your request for a Dean's Letter or Medical School Preformance Evaluation (MSPE) and where it needs to be sent to:

If you need a transcript, then send $5 (check payable to: University of Virginia) and your request with your full name, student ID (social security number), signature and a stamped envelope (with the complete address to where the transcript should be sent) to:

If you need a reduced copy (8.5 " x 11") of your diploma certified, send the diploma copy and a stamped envelope to:

Need a replacement for a damaged or lost diploma? Effective July1, 2001 the cost will be $50.00 plus mailing fees (within U.S. the cost is $9; International the cost is $19). Contact the Diploma Coordinator.
