Do you need a letter certifying that you are a current student? Do you need a licensure form filled out as a graduate of the School of Medicine? Do you need an Insurance Form filled out to verify that you are a "good student" to receive your discount?

You can print a request for certification form from the UVa Registrar's web site.

You can mail your request for certification form or licensure form and a stamped return envelope to:

Need a replacement for a damaged or lost diploma? Effective July1, 2001 the cost will be $50.00 plus mailing fees (within U.S. the cost is $9; International the cost is $19). Contact the Diploma Coordinator.
