Sexually Transmitted Diseases (STD's):
PID: Rocephin (Ceftriaxone) 250 mg IM x1 + Doxycycline 100 mg po BID x 14 days, Cefoxitin 2.0 g IM + Probenecid 1.0 g at same time + (Doxycycline or Erythromycin 500 mg po QID x 7 days)
GC (urethritis, cervicitis, proctitis, prostatitis, pharyngitis...): Ceftriaxone 125-250 mg IM x 1 + (Doxycycline 100 mg po x 7 days or Tetracycline), Ceftriaxone + Azithromycin (for C. trach.) 1.0 g po x 1, Doxycycline + (Spectinomycin 2.0 g IM or Cipro 500 mg x 1 or Floxin 400 mg po x 1 or Unasyn 1.5 g IM x 1 + Probenecid 1.0 g po x 1)
NonGC Urethritis or Cervicitis (Chlamydia~50%): Doxycycline 100 mg po BID x 7 days, Azithromycin 1 g po x 1, Erythromycin 500 mg po QID x 21 days, Floxin 300 mg po BID x 7 days; Note - add Ceftriaxone 125-250 mg IM x 1 for possible GC infection
Candida Vaginitis (white thick odorless cheesy discharge, wet mount, KOH prep): Monistat 3 Vaginal Supp. (200 mg qHS x 3), Monistat 7, Gyne-Lotrimin, Fluconazole 150 mg po x 1
Trichomonas (purulent foul smelling discharge, frothy, wet mount, pH 5-7): Flagyl 2.0 g po x 1, Flagyl 250 mg po TID x 7 days, Flagyl 500 mg po BID x 7 days, Gyne-Lotrimin (safe with preg.) 200 mg intravag. qHS x 3 days or 100 mg qHS x 7 days
Bacterial Vaginosis/Gardnerella (fishy odor, clue cells): Flagyl 500 mg po BID x 7 days
Syphillis: Benzathine PCN G 2.4 million units IM x 1, Ceftriaxone 1.0 g IM QOD x 4 doses, Doxycycline 100 mg po BID x 14 days; Lymphogranuloma venereum (C. trach): Doxycycline 100 mg po BID x 21 days, Erythromycin 500 mg po QID x 21 days; Granuloma inguinale: Doxycycline, Bactrim; Herpes Simplex; Zovirax (Acyclovir) 200 mg po 5x/day x 10 days, 400 mg po TID x 10 days, Xylocaine 5% ointment
