Local Invasion and Lymphadenopathy

Extension of a mass beyond the thyroid capsule into adjacent trachea or muscle is highly suggestive of an aggressive malignancy. Likewise, the presence of local lymphadenopathy with any suspicious features, such as rounded shape, loss of fatty hilum, cystic change, microcalcifications, or irregular internal hypervascularity, should prompt FNA of the node and any associated thyroid nodules.
