If you are using ERAS only you do NOT need an Address List Form
Select the Address List Form that you need and follow the directions below for downloading the form to your microcomputer. You will use this form to submit residency program addresses to the Office for Student Affairs for mailing the Dean's letter. Follow the directions on the Address List Form.
Click on the Address List Form and it will download the Microsoft Word file to your microcomputer. The file (for example, addr1.doc) can be saved to the Windows/Temp directory on your C drive. For MAC users, you may have to use the "Find Function" to see where the file was saved. You will have to open the file (addr.doc) in Word.

Transcripts for non-ERAS programs should be ordered from the UVA Registrar. Have them sent directly to the programs.
